Abstract

Objectives: It was aimed to compare the postoperative data of pediatric cases who underwent ultra-mini percutaneous nephrolithotomy (Um-PNL) in the prone and supine positions.

Methods: The data of pediatric patients aged 15 years and younger who underwent Um-PNL for the treatment of renal stones in the Urology Clinic of Dicle University Faculty of Medicine between March 2012 and March 2016 were retrospectively analyzed. Demographic characteristics, perioperative data and postoperative data of the patients were recorded. In terms of the position given to the patient during the operation, the patients were divided into two groups as prone Um-PNL (Group 1) and supine Um-PNL (Group 2). 

Results: When the success rate was evaluated in the first month after the operation, it was seen that stone-free was achieved in 65 patients (81.3%) in Group 1 and in 34 renal units (85.0%) in Group 2. Additional interventions were applied to 6 patients in Group 1 and 3 renal units in Group 2. The number of patients who were stone-free after additional intervention was calculated as 71 (88.8%) in Group 1 and 37 (92.5%) in Group 2. There was no statistically significant difference between the two groups in terms of stone-free rates at the post-op 1 month and after additional intervention. Complications were observed in 32 (26.6%) of the patients in the study. The distribution of complications according to the modified Clavien scale was Grade 1 in 11 (9.2%) patients, Grade 2 in 12 (10.0%) patients, Grade 3a in 6 (5.0%) patients, and Grade 3b in 3 (2.5%) patients. was found in the form. There was no statistically significant difference between the prone and supine Um-PNL groups in terms of complication rate and degree (p=0.436).

Conclusion: In the treatment of stone disease in the pediatric age group, supine Um-PNL is an effective and safe method, similar to Um-PNL in the prone position.
