Abstract

Objective: To analyse the expression mechanism of CD71 and immunoglobulin A (IgA) in IgA nephropathy complicated with sepsis, using a laser confocal scanning microscope (LCSM)

Method: A sample of 69 patients with IgA nephropathy complicated with sepsis were selected as the experimental group. Additionally, 103 patients with IgA nephropathy but no sepsis were selected as the control group, and 35 healthy people were selected as the blank group. The fluorescence deposition intensity of renal tissue in three groups was measured by direct immunofluorescence assay (DFA). Haematoxylin-eosin (HE) staining was used to detect the positive expression and grey value of molecule CD71 and IgA in tissue samples. The Spearman correlation was used to analyse the fluorescence deposition intensity correlation between molecule CD71 and IgA.

Results: The results showed that the positive expression rates and grey values of CD71 in the experimental group were significantly higher than those in the control group and the blank group, while those in the control group were significantly higher than those in the blank group (P<0.05). Additionally, the positive expression rates and grey values of IgA in the experimental group were significantly higher than those in the control group and the blank group, while those in the control group were significantly higher than those in the blank group (P<0.05). The fluorescence staining deposition intensity of CD71 and IgA were Level 3 (+++) in the experimental group, Level 2 (++) in the control group, and Level 1 (+) in the blank group. There was also a significant positive correlation between the fluorescence deposition intensity of CD71 and IgA (R=7.392; P<0.001).

CD71 and IgA positive expression levels were higher in patients with IgA nephropathy complicated with sepsis than those with IgA nephropathy without sepsis and in normal renal tissue. The molecules CD71 and IgA were co-deposited in the mesangial cells, and CD71 could bind to IgA, resulting in the co-deposition of the two molecules.
