Abstract

Objective: Serum ischemic modified albumin (IMA) index combined with Fourier transform infrared spectroscopy (FTIR) was used to assess the oxidative stress injury in patients with type 2 diabetes mellitus (T2DM) complicated with early diabetic nephropathy (EDN), in order to provide a new way for the early diagnosis and treatment of diabetic nephropathy.

Methods: 106 Patients with T2DM were divided into simple diabetic mellitus (SDM) group (UMAER <20 μg/min, n=49) and early diabetic nephropathy (EDN) group (UMAER: from 20 μg/min to 200 μg/min, n=57) according to 24 h urine microalbumin excretion rate (UMAER), and 32 healthy volunteer subjects confirmed by physical examination were chosen as the control group. Attenuated total reflection Fourier transform infrared spectroscopy (ATR-FTIR) was used to detect the serum of the subjects, and A1650/A1550 values were recorded. The serum ROS of the subjects was determined by Elisa. The serum albumin and CK-MB, fasting blood glucose were measured, and Ischemia modified albumin (IMA) was measured by ACB method and the IMA index was calculated. 

Results: The age and sex of subjects were not significantly different among the groups, and there was no significant difference in diastolic blood pressure between SDM group and EDN group. There was no significant difference in the course of disease, fasting blood glucose and glycosylated hemoglobin between EDN group and EDN group. The results showed that the systolic blood pressure (SBP) of subjects in EDN group was significantly higher than that in the control group, the WHR, 8-OHdG, IMA index and A1650/A1550 in SDM and EDN groups were higher than those in the control group, and those in EDN group were highest among the different groups. 

Conclusion: The oxidative stress injury is involved in the occurrence and progression of diabetic nephropathy. A1650/A1550 detected by ATR-FTIR and IMA index can be used as new indexes to assess the oxidative stress injury of diabetic nephropathy.
